Garland Porterfiled, MD 3 ladiesWhat is breast augmentation? Breast implants are for women looking to enhance the size and shape of their breasts and can be used for reconstructive purposes. This procedure is becoming more and more popular as breast implant technology and surgical techniques advance. Women can achieve a more flattering, better-proportioned figure, and even enhance confidence and self-esteem.

What is the most common reason women choose to have breast augmentation? The most common reason for breast augmentation surgery is to increase the size of the breasts. The procedure is also done to balance unevenly sized breasts, or regain a more youthful shape after pregnancy, breast-feeding, or aging. This surgery is one of the most popular cosmetic procedures. 

What can it do for a woman? Breast augmentation surgery can significantly increase confidence by providing a more appealing appearance and thus improving self-esteem and self-confidence.

What is breast augmentation? Breast augmentation is achieved by making an incision and placing implants into the body. Options can include incision location, implant pocket location, and implant type.  The implant is a shell containing either saline or gel-like silicone. The shell itself is a rubber-like material called silicone elastomer. The FDA regulates the type of implants that may be used within the United States.

Who are the best candidates for breast augmentation surgery? Garland Porterfiled, MD augment2The best candidates for breast augmentation are women who want to improve their looks. Those who are physically healthy and realistic in their expectations may be good candidates. Before you decide to have surgery, think carefully about your expectations and discuss them with Dr. Porterfield.

What are my options for incision location? Three main options are available for incision location: under the breast (inframammary), in the armpit (axillary), and around the nipple (periareolar). The choice of incision used depends on whether this is a first implant or a revision surgery, type and size of implant being used, scarring, and other factors. Incisions are always made where the scarring will be least noticeable. Discuss these options with Dr. Porterfield as each have advantages and disadvantages.

Where is the implant placed?

  • In a subglandular placement, the implant is placed under the breast tissue and above the muscles of the chest wall. The thickness of your breast tissue and it’s ability to adequately cover the implant is one factor that determines whether this placement is used. Subglandular placement may make surgery and recovery shorter and less painful.   This placement may result in more palpable implants, more capsular contracture, and more difficult imaging of the breast with mammography. 
  • In a partial submuscular placement, the implant is placed under the breast tissue and partially under the pectoral and other chest muscle. The surgeon will cut the pectoralis major muscle and slip the upper part of the implant under that, while the lower part of the implant is placed above the muscles and beneath the breast. This placement may result in less palpable implants, less capsular contracture, and easier imaging of the breast with mammography.
     
  • In a complete submuscular placement, the implant is placed completely under the muscles and connective tissue called fascia. The upper portion of the implant is behind the pectoralis and serratus muscles, while the lower part is under connective fascia connecting those muscles. Submuscular placement may be more painful, and may make it more difficult to have some reoperation procedures than the subglandular placement.

What are the types of implants used? There are saline filled and silicone gel filled implants. Additionally, many different sizes and shapes exist for both types. Both use an outer shell or sack made of silicone elastomer, which may have a smooth or textured surface.  There are advantages and disadvantages to each type, and certain situations may warrant the use of one rather than the other. Patients should discuss the options with Dr. Porterfield and carefully weigh all of the facts before making a decision.

  • Silicone Breast Implants
    Silicone breast implants consist of a silicone elastomer shell filled with silicone gel.Garland Porterfiled, MD augment3 This type of implant was taken off the market in 1992 (except for use in women undergoing breast reconstruction) because of reports of women developing autoimmune diseases and other problems. In 1997, the federal government asked the Institute of Medicine of the National Academy of Science to examine the complications of silicone breast implant surgery. The institute concluded that "Evidence suggests diseases or conditions such as connective tissue diseases, cancer, neurological diseases or other systemic complaints or conditions are no more common in women with breast implants than in women without implants." Following a rigorous review process, the FDA approved the use of silicone gel-filled breast implants in 2006 for reconstructive breast surgery on women of any age, and for cosmetic augmentation surgery on women 22 years old and older.  The FDA and implant makers recommend patients have magnetic resonance imaging (MRI) performed regularly to detect any leakage of silicone gel. The cost of these MRIs may not be covered by your health insurance, and you may have to pay out of pocket for them.
  • Saline Breast Implants
    Saline breast implants were available before the ban on silicone breast implants by the FDA, and became the most popular choice after that time and before silicone implants were reintroduced. Saline implants contain sterile salt water as the filler material, the same main ingredient in body fluids. If a saline implant ruptures, the implant deflates and the body absorbs the saline harmlessly. This may leave an asymmetrical appearance until the implant is replaced. Saline implants may not replicate the look and feel of a breast as well as silicone implants can, but many women choose them because of the safety concerns over the use of silicone. You must be at least 18 years old to undergo cosmetic breast augmentation with saline implants. This is a different minimum age than for silicone implants because the risks of the implants are different. There is no risk of silent rupture with saline implants and saline implants do not require having regular MRIs.

What does the procedure involve? The procedure usually lasts about one hour or less and consists of three main phases: preparation, anesthesia, and surgical procedure. Right after the surgery, the patient is awakened and moved to a recovery room where nurses monitor vital statistics for usually about 1 hour. You may feel a number of sensations during this time, including pain or nausea, The staff is well trained and experienced in making you comfortable and ensuring your speedy recovery. They can provide you with pain relievers, anti-nausea medications, and warm blankets if you need them. You are then allowed to return home, and you may feel somewhat tired and sore for several days following the operation. Your breasts may remain swollen and sensitive to physical contact for as long as a month. The full results of your augmentation may not be visible until your breast tissue (and muscle, if the implant has been placed submuscularly) adjusts.

Do I receive information about my implants? Yes. After surgery, you will receive a card containing information about the implants. This is called the device card. Keep it in a safe place because you may need it in the future. The card gives information about the brand, type and lot numbers of your implants.

Are there post surgery checkups? Yes. You will return to the office in approximately one week. Your next visit is typically fourteen days from your surgery date to learn breast exercises. After this time, visits are scheduled depending upon your needs.

What are the risks involved? Breast augmentation is relatively straightforward, but as with any operation, there are risks and complications associated with this procedure. Dr. Porterfield is both well trained and well experienced in this surgery and knows how to keep the risks as low as possible, while ensuring the very best outcome and speedy recovery.  While the majority of women do not experience complications, you should discuss them with Dr. Porterfield to make sure you understand the risks and complications of breast augmentation.
