Where is the implant placed?

a. In a subglandular placement, the implant is placed under the breast tissue and above the muscles of the chest wall. The thickness of your breast tissue and it’s ability to adequately cover the implant is one factor that determines whether this placement is used. Subglandular placement may make surgery and recovery shorter and less painful.   This placement may result in more palpable implants, more capsular contracture, and more difficult imaging of the breast with mammography.

b. In a partial submuscular placement, the implant is placed under the breast tissue and partially under the pectoral and other chest muscle. The surgeon will cut the pectoralis major muscle and slip the upper part of the implant under that, while the lower part of the implant is placed above the muscles and beneath the breast. This placement may result in less palpable implants, less capsular contracture, and easier imaging of the breast with mammography.

c. In a complete submuscular placement, the implant is placed completely under the muscles and connective tissue called fascia. The upper portion of the implant is behind the pectoralis and serratus muscles, while the lower part is under connective fascia connecting those muscles. Submuscular placement may be more painful, and may make it more difficult to have some reoperation procedures than the subglandular placement.

WHAT IS ASILICONE GEL-FILLED BREAST IMPLANT?
A breast implant is a sac (implant shell) of silicone elastomer (rubber) filled with silicone gel. This sac is surgically implanted either under your breast tissue or under your chest muscle. Dr. Porterfield will give you his recommendations as to the very best placement for you.

This type of breast implant material  holds together uniformly (no leaking) while still retaining the natural give of breast tissue. It does this because of a cohesive, gelatin-like substance that acts as a solid rather than a liquid.

WHAT GIVES THE BREAST IT’S SHAPE?
The breast consists of milk ducts and glands, surrounded by fatty tissue that provides its shape and feel. Situated beneath the breast are muscles. Certain factors may cause the skin to stretch. This then may cause the breast to droop or sag. These factors include pregnancy (when milk glands are temporarily enlarged), rapid weight loss, and the effects of gravity as you age.

WHAT CAN DR. PORTERFIELD DO FOR YOU?
It is important to realize that implants are used to make the breast larger.  However, the implants alone may not adequately lift the breast, or correct the effects of pregnancy, weight loss, or skin stretching. It is important to discuss with Dr. Porterfield your hopes and wishes for your best look. Based upon your wishes and his expertise, Dr. Porterfield may suggest additional procedures at the time of the breast augmentation, such as a breast lift to improve shape.
